Severity Matrix

  • SEV1: customer-facing outage or severe data integrity risk
  • SEV2: major degradation with partial customer impact
  • SEV3: localized degradation or operational risk without broad impact

Use the highest supported severity. If the evidence is incomplete, report uncertainty instead of downgrading silently.
